Position Description

$37.00- $55.50 per hour (Salary based on years of experience) plus weekend differentials where applicable.

Westminster Place - Nursing Supervisor - Part Time - (7am-3pm) – Only Every Other Weekend (Saturday, Sunday) - Skilled Nursing Department.

Essential Functions

  • Works collaboratively with team to support Presbyterian Living's person centered care and Operation Excellence programs.
  • Insures adherences to all State and City regulations pertaining to Assisted Living. Maintains the documentation required for two state surveys.
  • Participates the inter-disciplinary service plan conferences.
  • Supervises and directs the nursing staff, CNAs and any other nursing personnel assigned to Skilled Nursing.
  • Assists in orienting and evaluating new nursing personnel and temporary personnel on assigned units.
  • Assists with performance evaluations of staff.
  • Counsels staff as needed and recommends disciplinary action to Director of Skilled Nursing or initiates disciplinary action personally when the situation warrants.
  • Maintain all documentation in the Wellness office.
  • Coordinate resident medical appt. with physicians, transportation and volunteers.
  • Participates in direct patient care when staff is otherwise engaged, especially as regards response to med administration and resident care and emergencies.
  • Enters pertinent resident information, including admission, discharges, and condition changes, on the written 24-hour report as well as the 24-hour resident status sheet.
  • Communicates changes in resident condition to resident’s doctor in a timely fashion while taking appropriate action in urgent and emergent situations.
  • Develops s a collaborative relationship with representatives from other departments to coordinate person centered care.
  • Documents pertinent information on inter-shift report and gives verbal report to oncoming shift staff.
  • Participates in meetings each month to disseminate information and discuss departmental issues and concerns.
  • Communicates with families to keep them informed of changes in patients’ conditions and/or patient’s needs.
  • Note: Please note that the responsibilities and tasks outlined in this job description are not exhaustive. Additional duties may be assigned as needed to support the goals and objectives of the organization.

Education: Graduation from an approved registered professional nursing program.

Qualifications

  • Active, current Illinois license to practice professional nursing.
  • CPR Certification Required - Accredited by American Heart Association.
  • One year as an RN in a hospital (general care) or long-term care setting with adults preferred.
  • Genuine interest in working with older adults.
  • Knowledge of aging changes and common health problems of geriatric patients.
  • Good communication skills.
  • Ability to direct nursing assistants.
